Tattoo Release Form

-This needs to be completed on the day of your appointment, prior to service.

-Please have your phone and photo ID available at the time of service.  This form will be required for each tattoo appointment.

-If you have an appointment scheduled and want to complete the form in advance, please limit it to one day prior to your appointment so it is easy for us to track it down.

-Please read the Pre-Service Instructions if this is your first tattoo at Contender Tattoo Studio.
